We have recently made some changes to our Planning applications & forms.  All of the applications have been consolidated into a single form and can be submitted online for ease & convenience.  You can also pay for the application in a few quick & easy steps.  The two payment options are by check or credit card. Please note that for online credit card payments, a 2.80% processing fee will be added to the total review fee. Also, if paying by check, your application will not be processed until payment is received by our office.  

Please use this form for all planning reviews you are applying for.  You will need to fill out separate application forms if applying for multiple projects that have different parcel numbers.

A Certificate of Appropriateness review is not required if a building permit is not required.

For Wireless Telecommunications, fill out an application for NEW FACILITIES and SUBSTANTIAL MODIFICATIONS to existing facilities. Not-Substantial Modifications may not require an application.

A Conditional Use request requires a detailed narrative (project description) and site plan graphics. For the detailed narrative, would you prefer to attach a PDF document or enter the information directly into this application form?

Provide a detailed narrative that addresses specifically how the following six criteria and standards are met:

Please attach a DETAILED NARRATIVE that answers the following conditional use criteria and standards: 1. That the establishment, maintenance or operation of the conditional use will not be detrimental to or endanger the public health, safety or general welfare. 2. That the uses, values and enjoyment of surrounding properties for purposes already permitted in the district will be not be substantially impaired or diminished by the establishment, maintenance or operation of the proposed conditional use. 3. That establishment of the conditional use will not impede the normal and orderly development and improvement of surrounding properties for uses permitted in the district. 4. That adequate utilities, access roads, drainage and other necessary improvements have been or are being provide. 5. That adequate measures have been or will be taken to provide ingress and egress that will minimize on- and off-site traffic congestion. 6. That the conditional use complies with all other applicable regulations of this zoning ordinance. Finally, provide details that will help with decisionmaking about the operation of the use. Include hours and days of operations, seasonal operation, shipment/delivery requirements and hours, parking requirements, drop-off zones, queuing lines, height oand width of existing and new structures (for wireless telecommunications, site plan details, etc.

Sworn statements from the responsible party attesting that colocation would not result in the same mobile service functionality, coverage, and capacity, or is technically unfeasible. A short-term rental is a residential dwelling that is offered for rent for a fee for fewer than 29 consecutive days. Short-term rentals in R1 and R2 districts may not offer more than one rental stay in 7 consecutive days. In all other districts, the minimum rental period is one day. Do you understand the referenced stay requirements?

Please attach a DETAILED NARRATION that provides specific hardship evidence based on Wis. Stat. 62.23(7)(e)7. Criteria includes the following: A. Provide proof that the dimensional, physical or locational requirement of the code is an “unnecessary hardship” by demonstrating that strict compliance with the zoning ordinance would unreasonably prevent the property owner from using the property for a permitted purpose or would render conformity with the zoning ordinance unnecessarily burdensome. B. Provide proof that the unnecessary hardship is based upon circumstances unique to the property rather than considerations personal to the property owner. C. Provide proof that the unnecessary hardship was not created by the property owner.
